Restore Mobile Therapy Low Vision Services

Restore Mobile Therapy is excited to be partnering with the Lion’s
Low Vision Centers in Connecticut!

ALL devices that are given to you during a visit are 100% FREE, and provided by the Lion s Clubs of Connecticut.

Steps to getting set up with your OT

Step 1

Visit or contact your eye care
professional to complete a
prescription which includes your level of vision impairment and an order for an Occupational Therapy Low Vision Evaluation.

Step 2

Prescription is then faxed by
your eye care professional to
Restore Mobile Therapy. Fax: 203- 347-2572.

Step 3

Restore Mobile Therapy will
then contact you to go over
your insurance benefits prior to scheduling the initial visit. Your OT will then call to schedule a visit.
